This is a friendly reminder to parents/guardians and students that proof of a physical exam is required to participate in athletics at Memorial. A student cannot participate in tryouts, practices or games, unless there is record of a current (within the last 2 years) physical on file. Many physicals have expired or will be expiring in the Fall 2009. Physicals may be faxed to Memorial at 772-4597.

A permission slip/athletic contract (available in the office) must be filled out and signed by a parent/guardian and the athlete before participation in athletics.
